This 34-year-old healthy woman wanted larger, fuller, natural appearing breasts. She wanted to avoid having to cut the pectoralis major muscle of her chest, as is traditionally done in most breast augmentation cases. We therefore opted for a composite breast augmentation in this case. Smooth, round, moderate projecting 375g silicone implants were placed through an inframammary fold incision. The upper pole and cleavage area then received some fat graft harvested from the abdomen. This allowed for a natural, full and soft upper pole and cleavage area without having to cut the pectoralis muscle. Leaving the muscle intact is desirable because ot avoids any loss of chest muscle power, and also eliminates implant animation which is the deformation of the implant with strong muscle contraction.
